Job overview
Members do best when they have someone they trust who understands their community. As a Remote Community Health Worker you will support members through outreach, plain-language health education, social needs screening and connections to community resources, working alongside the member's care team.
This is a community health work role. It is distinct from clinical care management, because the focus is community-based outreach, health education and culturally responsive support rather than clinical care planning. You work with health plans, accountable care organizations (ACOs), value-based care groups, community health programs and social services agencies. The specific program and scope depend on the team you join.
This opportunity may suit people searching for community health worker jobs, remote CHW jobs, work-from-home community health jobs, or remote community outreach jobs.
Experience Required
Previous experience in community health work, health education, community outreach or culturally responsive member support is required. Community Health Worker (CHW) certification is preferred but not required by all employers. Bilingual and multilingual candidates are often especially valued. Confirm requirements with the employer before applying.
Structured Remote Work with Predictable Hours
Community health work runs on scheduled outreach and follow-up, usually weekday daytime, with some programs adding evening hours to reach members. Both full-time and part-time positions are offered, with no in-person requirements. Discuss your preferred schedule in your interview.
Member safety, scope of practice and privacy first
This is a community health work role, not a clinical one. If a member reports acute symptoms, safety concerns or other urgent needs, you follow the employer's escalation protocol immediately, including directing the member to call 911, contact the 988 Suicide & Crisis Lifeline or seek emergency care as appropriate. You will handle member information, so employers expect HIPAA compliance and strict confidentiality.

- Provide health education in plain language on conditions, medications and healthy behaviors
- Support members with culturally responsive approaches that respect language, culture and community
- Screen members for social needs such as food, housing, transportation and social support

- Support members in understanding care plans and provider instructions, and encourage health behavior change
- Coordinate with the member's care team, including the PCP, care manager and social worker
- Document member contacts accurately, escalate unmet needs and follow emergency escalation protocols
