Job overview
Patients living with chronic conditions do better when they feel confident managing their health day to day. As a Remote Chronic Care Coach you will help patients understand their condition, build self-management skills, set condition-specific goals and stay engaged in their care.
No prior chronic care coaching experience is needed. Paid training covers chronic condition education, behavior-change coaching, coaching platforms, documentation and HIPAA basics. The specific program, conditions and scope depend on the team you join.
This opportunity may suit people searching for chronic care coach jobs, remote chronic care coaching jobs, work-from-home health coach jobs, or no experience healthcare coaching jobs.
No Experience Required: We'll Train You
No prior chronic care coaching experience is required, and a certification is not required for some roles. Employers provide paid training on chronic condition education, behavior-change coaching, monitoring support and HIPAA, and you learn the work step by step. Some employers may prefer a CDCES, health coaching or chronic care certification, so confirm in your interview.
Structured Remote Work with Predictable Hours
Coaching runs on scheduled patient sessions and follow-ups, usually weekday daytime, with some programs adding early morning or evening sessions to match patient availability. Both full-time and part-time positions are offered, with no in-person requirements. Discuss your preferred schedule in your interview.
Non-clinical coaching role, scope of practice and privacy first
Chronic Care Coaches do not diagnose, treat, prescribe or give clinical advice. Clinical concerns, medical questions and situations that need clinical judgment are escalated to clinical teams per the employer's protocol. Symptom changes are always escalated to the clinical team. You will handle names, contact details and health-related information, so employers train you on HIPAA awareness and expect strict confidentiality.
